Risk Assessment Form – Part B

Continue on separate sheet if necessary

1 – complete separate table for each activity     2 - from hazard list in Part A     3 - from risk matrix in Part A
Activity1 Each individual activity you are proposing:

Locationwhere this activity will take place:

Dates / times:

Hazards2
List what could cause harm from this activity
e.g. falls from height, trip hazard, fire, etc.
Who exposed
List who might be harmed from this activity
e.g. staff, contractors, contributors, public, etc.
Risk3
For each hazard, decide level of risk as if you were to do the activity without your controls
Control measures
For each hazard, list the measures you will be taking to minimise the risk identified
e.g. appointing competent persons, training received, planning and rehearsals, use of personal protective equipment, provision of first aid, etc.
Risk3
For each hazard, decide level of risk once all your controls are in place
Assault by person

ALL
Remote - Very Severe
Avoid arguments with any members of the public. Prepare the first-aid kit
Remote - slight
Cold environment

ALL
Possible - severe
Warm clothing will be advised to be worn when the actors are not acting. If an actor is extreme cold, we will consider to film the scene in a warmer day which this actor has involved in
N/a
Slippery surface

ALL
Possible - negligible
Skid shoes or stout footwear is advised where possible. When walking outside, walk slowly and care should be made by all actors
Remote-negligible
Contact with cold surface

Homeless actor
Remote - negligible
Actor must ensure that he is warm enough and he avoids touching any cold surfaces
Remote - negligible
Nuisance noise/vibration
ALL
Very unlikely - moderate
Keep the filming place at least 5 meters away from the noise
N/a
Physical fatigue
Actors and actresses
Remote - very severe
Medicine should be brought and a phone should be brought to ensure support can be requested if needed
N/a
Stress/anxiety
Actors and actresses
Remote - negligible
Stop filming and take a rest if the actor is stressful, talk to him and make him calm down

The finale script

After we gave our script teacher and she gave back us of the feedback, we changed a lot and this may be our finale script:
REVENGE
------by Cynthia,Gigi and Melissa

FADE IN:

EXT. A DARK ALLEY-RAINY NIGHT

A very common alley, dirty and dim. There was one street lamp shining beside the entrance of the lane illuminating a big bin stuffed with trash. A big shadow in cast in the alley. Audience can see something hiding in the dark but can't see clearly.

The sound of filling bullets in a car-triage can be heard, clear, slow and rhythmic……(continue)

CUT TO:

THE SHADOW FILLED ALLEY

ALLEN, a 22-year-old killer, tall but not muscular, leaning on the wall filling his magazine. He was wearing a black coat and black hat, can’t see his face clearly. He finished filling and loaded his handgun, walks out the alley.(sound stop).

CUT TO:
EXT. STREET-RAINY NIGHT

A homeless boy curled up in his blanket on the pavement.

Allen walks by and stops, takes out his wallet, took out an old cross necklace which seemed to belong to a woman and puts it into his pocket. Then he gives his wallet to the boy.

A  HOMELESS BOY, around 11~13, looks up and takes over the wallet.

BOY
(Pleasantly surprised and a bit confused)
Thank you very much sir! May God bless you!

Allen gazes at the boy in silence for a moment, then bowed and touched boy's hair.

ALLEN
(Gentle and a bit sorrowful)
May God bless you too.

Allen leaves and walks away along the late night street with no people.

DISSOLVE TO:

INT. PRIESTESS BAR-A COVERT PARLR-NIGHT

Two cups of colorful cocktail are placed on the table. The one before Cassandra was almost over, the one before Allen was barely touched. Light was soft yellow and the music of bar could be heard indistinctly.

CASSANDRA, a famous hacker and also a gorgeous blonde beauty, wearing a poppy in her hair, was a bit stunned by what she heard. A smile disappeared from her face.

CASSANDRA
You must be kidding, darling.

Allen stayed in silent, shaking his glass gently.

CASSANDRA
He is your foster father.

ALLEN
Yes.

CASSANDRA
And you want him dead.

ALLEN
Yes.

CASSANDRA
You are insane.

ALLEN
I know that.

(A long period of silence.)

CASSANDRA
(Speak suddenly)
Did I tell you insanity is my favorite?

Allen still not say anything, but he stops shaking the glass.

CASSANDRA
(Lied back, smiled)
Tell me, what do you want, sweetheart?

She picks up the glass and drinks the last drop of cocktail.

CASSANDRA
How about, the control of Raven Manor's security system?

Allen looks up, his eyes looking at Cassandra’s eyes, Cassandra just replies a smile to him.

ALLEN(v.o.)(CONT'D)
Sometimes the past is more stubborn than you think. No matter how far you run, how fast you can be, it can always catch you. Like a snake, like a shadow, like a ghost.

My father was a dye-in-the-wood bastard. Since I can remember, the only time I would see him is when he come for money.

My mother was the one who gave me life, raised me, and built my happiness childhood. I still remember how she looked like in that lilac dress--- her favorite one, how she called me "my dearest boy" and the fragrance of her skin.

I had seen my father beat her. From that time, I swore I would protect her……however…

CUT IN:

BLACK SCREEN

Sound of gun shots.

DISSOLVE IN:

ALLEN'S HOUSE-LIVING ROOM-DUSK

Allen's mother's body was lying on the floor, she was tidied and eyes closed, adjusted to a serene position. Allen's father was lying not far away from her, face down and five bullets hole on his back, not tidied. Blood turned the carpet red.

ALLEN(v.o.) (COUNT.D)
Everything happened so fast, I didn't even realise what happened when I saw the blood. I was so stunned, which strangely make me calm.

I walked past my father, saw his body, didn't feel anything inside. When I kneeled beside my mother, touched her hands-they were ice-cold, I suddenly realised one fact: I’d lost her, forever.

DISSOLVE TO:

CHURCH-PARENTS' FUNERAL-DAY

Allen was sitting on a chair in corner, guests were talking to each other quietly.

GUESTA/B, Guests invited to join the funeral.

GUEST A
I heard they were murdered by mafioso.

GUEST B
Is that for real?

GUEST A
His father stole their goods and......

GUEST B
Shhhhh! The boy is looking at us!

Guest B walked to Allen.

GUEST B
Be strong, my child. We will be with you.

Guest B tried to touch Allen's hair, but he pushed her and ran away.

CUT TO:

CHURCH-BENCH NEAR DOOR

Allen hid under the bench, after a while, he heard someone was talking and walked in.

PRIEST, the priest who directed the funeral.

PRIEST
......when I found him, he was sleeping with his mother's body in blood. Until today, he still refuse to talk.....what a poor child!

Priest saw Allen under the bench.

PRIEST
Why you are here? Come out.

Priest turned to Syrus

PRIEST
Here is the child.

SYRUS, a professional killer, 29-years but looks two years younger than actual. Wearing camel coat and quadrille scarf. Mild and grateful.

Syrus crouchedlooked deep into Allen's eyes and then took him out. He held Allen in arms.

SYRUS
(Smile)
Nice to meet you, little Allen.

ALLEN(v.o.)
I should appreciate him. He adopted me, looked after me, taught me well…

Little Allen looks up to Syrus’s face, just like he seeking for the hope.
(PS: the highlight part is that we might film that part or maybe we will delate it. We still unsure that we might film that part therefore we highlight it.)
